Identity theft can affect your credit, create fraudulent debt or false medical records, and much more.

Identity theft is a crime that can happen to anyone and involves your personal information being stolen through methods as simple as stealing your documents or as sophisticated as “phishing” or spyware.

Requirements

1. Please complete an Identity Theft Packet Form and print it.
   — This form provides us the information we need to make an offense. It must be filled out with as much information as possible.
2. Please have the form signed and notarized or witnessed.
3. Please fax, mail, or scan and email the completed form.
